Here's one: a Machine cult hiding in the tunnels beneath a car factory near the mountains, based around the worship of a massive collection of V8 engines they hoard in a collapsed mountain tunnel. They are very territorial, and make use of homemade guns and weapons. Each has a symbol from a car company branded on their neck.

Here's another: A fanatic religious militia dwelling in the catacombs beneath a major city. They are led by an ex-bishop that perverted the beliefs of Christianity with his own insane ideals, much of that being human sacrifice and self-mutilation.

You could have some sort of Neo-Roman Empire people that want to try and mimic the old governmental and military systems of the Romans. Also based out of Venice you could have a group of drug smugglers using old Venetian ports as their base of operations. Also you could have a group be the remnants of the Italian military that want to try and retake the country through military conquest. After typing this out I realized these are all ideas from Fallout 3 and New Vegas but they could still probably work for you.

But I don't like Fallout. I hate your ideas. I actually like them. Specially the drug part.
