Is there a reporting format?

Hi there!

I looked through the report section of this forum, but I couldn’t find any form of standardised reporting format. As far as I have understood it, you are pretty much allowed to form your report any way you like. I’m not saying that this is a bad way of doing things, but I feel that it is open for misunderstandings? For example, is there any requirement that you got to possess video evidence when you report a hacker? As they use to say, a picture can say more than a thousand words.

I’m just asking those questions to know whether I have missed something, or if there are things that could be further improved.
 
That would explain why I remember seeing something like that at one point, but I can't find it anymore.
 
could we also send a staff member to their forums to tell them what they dont have or what they are doing wrong?
 
Since I have soon a year of experience as a staff member on a big server myself, I know that the simple yet standardised format is very useful and efficient. Something like this would come to good use:

I was never sent over here by someone else, I came here by free will as a friendly gesture. It would of course still be very nice to get input and feedback from you guys.
 
The correct way is putting the username and the name of the violation in the title, with evidence and additional explanation if needed :3
